A free program that gives first responders immediate access to your critical medical information when every second counts.

What is the File of Life?

File of Life is a free Southern Berkshire Ambulance Squad program that can save valuable time in the event of an emergency. Participants complete a medical information form that contains their pertinent medical history, current medications, allergies, emergency contacts, and other critical details.

The completed form is placed on the refrigerator — the preferred and most recognized location — or somewhere easily visible and accessible to first responders. When our crew arrives at your home, they know exactly where to look.

Who should use it?

The File of Life is especially valuable for older adults, individuals with chronic conditions, and anyone who lives alone. It ensures that even if you are unable to communicate during a medical emergency, our crew has the information they need to provide the best possible care.

How to get one

File of Life kits are available free of charge. Call us at (413) 528-3632 to request one. If you already have a kit but need to update your information, download and print a fresh insert below.
